Use Parchment to order an official transcript or obtain a personal copy. From your account, you can send records securely to colleges, employers, scholarship programs, athletic organizations, or other recipients.

Complete the OUSD Transcript/Records Request Form. The district’s records page provides the current form and instructions for submitting it by email, fax, or mail.

  • Also use this process for immunization records.
  • OUSD cannot issue replacement diplomas.
