Organizations wishing to use OUSD facilities should obtain, complete and return an “Application and Permit for Use of School Facilities” form linked above with the button. Please complete forms and submit them to the site principal, or designee, who will verify if the facility requested is available. If so, they will then sign the form and forward to the Use of Facilities office at the OUSD District Office. Applications for Fred Kelly Stadium can be obtained at the Use of Facilities office. All applications must be submitted not less than 15 days nor more than 90 days prior to the requested use date.
